Incident Reports in JACK let you quickly capture everything you need at the time. Start this once you've ensured everyone involved is safe. The details you record here will support a post-incident investigation, so the more complete they are, the better.
Before You Start
Try to have this on hand before you begin, since some of this information may come from people who may leave the site shortly after the incident:
When and where: the date, time, and exact location of the incident.
Who was involved: full name, contact details, and their relationship to the business (worker, subcontractor, visitor) for anyone injured or involved.
What happened: a clear, chronological account of the activity taking place and what immediately caused the incident or near-miss.
Equipment and hazards: any machinery, tools, or substances involved, and the extent of any property or environmental damage.
What was done about it: first aid or emergency response provided, and control measures taken or planned to prevent it happening again.
Witnesses: contact details and a brief account from anyone who saw it happen.
Recording an Incident
Navigate to Jobs > select the relevant Job > Safety

Then click on Incidents and click the
button.
You can select from the 10 pre-set incident types, then record the Incident Details, the corrective actions implemented, and whether emergency services were called, with the specifics if so.
Scroll down to add People Involved and upload any Photos or Documentation.
Fill in as many sections as you can, with as much detail as possible, especially while witnesses are still on site.

Where to Find Recorded Incidents
You can view Incidents in two ways. This is within the specific job or via the global view once they are saved
Job Level:
Click on Jobs > Job Name > Safety > Incidents.
The job-specific incident appears straight away in this job's Incidents list and also in the global view by selecting Safety in the main menu. Here you will find the Incident ID, Type, Date, Time, and who recorded it for the Job.
Global Level:
Click on Jobs > Job Name > Safety > Incidents.
Here you will see Incident ID, Type, Date, Time, and who it was Recorded By. The only difference from Job Level is that you will see All Jobs.
These reported incidents can then be reviewed at any time for management review and/or any corrective actions as needed.
